Community of Practice on Transition

The Transition Improvement Grant (TIG) developed a Community of Practice (CoP) on Transition for educators as a tool to exchange information and gain knowledge to improve transition practices for students with IEPs. 

The CoP on Transition is focused on three main areas:

  • Rural Transition Practices
  • Urban Transition Practices
  • Suburban Transition Practices

The CoPs on Transition focus on topics of interest generated by each individual group with a shared leadership within the group.  The CoPs then set up meeting dates to join one another in a group learning dynamic either face to face or virtually.  Participants of the CoP have collaboration time to share areas of concern, new ideas, best practice resources, innovative practices, and ways to implement these transition practices in their home district.  A CoP also provides opportunities to build relationships with individuals who share the same desire to increase post school outcomes for students.
