Appropriate Measurable Postsecondary Goals
IEPs are annually updated and based upon an age appropriate transition assessment, transition services, and course of study, that will reasonably enable the student to meet those postsecondary goals, and annual IEP goals related to the student’s transition service needs. There also must be evidence that the student was invited to the IEP team meeting where transition services are to be discussed and evidence that, if appropriate, a representative of any participating agency was invited to the IEP team meeting with the prior consent of the parent or student who has reached the age of majority.
The Postsecondary Transition Plan (PTP) online application was developed by the Department of Public Instruction to assist districts in collecting data to meet the requirements of Indicator 13. The PTP is a part of the IEP and must be discussed during the IEP of students in transition age.

- Students must be invited to the IEP meeting (they are not mandated to come, but they must be invited). If a student does not attend, his/her interests and preferences must be considered. Documentation of this is required.
- Transition goals must be incorporated in IEPs for students age 13 turning 14 during the timeframe of their IEP and older.
- IEP must include instruction and community experiences.
- Transition goals must reflect both academic and functional achievement.
- Parents/Guardian must be notified that transition goals and objectives will be discussed at the IEP meeting.
- The district must invite outside agencies to the IEP meeting that are likely to be responsible for providing or paying for transition services during the timeframe of the current IEP (consent is needed).
- Transfer of Rights begins not later than one year before the student reaches the age of majority (age 18).
